Please follow these easy steps:

  1. Browse or search the test catalogue
  2. Click on the test of interest 
  3. Click on “Add to Test Selection” ( To view all tests being requested, click on "My Test Selection" at the top right of the page)
  4. Once all desired analyses is selected, click "My Test Selection" at the top right of the page and select "Go To Step 2"
  5. Provide the requested demographics and billing data (Note: if you are a returning user ordering healthcare provider information may be self-completed if using the same computer as previously)
  6. Click "Go To Step 3" to generate your customized requisition and print it out.
  7. Please print the required patient family and medical history information
  8. Send us your samples with your individualized requisition form

The following information should be included on all requisitions in order to ensure accurate results in a timely manner:

  • Patient demographics
  • Ordering physician/ healthcare provider demographics & contact information
  • Family history information
  • Testing requested
  • Signed, completed consent form 
  • Prefered payment option - checking the appropriate box. Please see the payment options and required forms listed below. If you have unique payment requirements, please contact us to discuss further.

Important: For all countries outside of Germany, we require a written and signed statement including the complete address and name of the person or institution who will cover the costs of the analysis.
